The Administrator also confirmed that the forms should include the census and staffing numbers but was unaware of the need to include actual hours worked per shift. The facility's policy on Staffing & Scheduling, last reviewed in June 2022, mandates compliance with CMS staffing requirements and the posting of staffing information at the beginning of each shift, but it appears this was not adequately followed.
