Summary

The facility failed to ensure that required PASRR (Preadmission Screening and Resident Review) Level I and Level II screenings were completed for two residents with mental health diagnoses. For one resident admitted with diagnoses including unspecified schizophrenia and bipolar disorder, there was no evidence of a PASRR Level I screening in the medical record from the time of admission through the date of review. The Director of Nursing confirmed that the PASRR Level I screening was missed for this resident. For another resident admitted with a diagnosis of unspecified psychosis, the pre-admission PASRR Level I screening indicated a positive result for serious mental illness, which required a Level II mental health evaluation.
