Summary

The facility failed to maintain documentation of a Level I preadmission screening and resident review (PASRR) assessment for a resident with serious mental illness and intellectual disability diagnoses. Record review showed that the resident had diagnoses including post-traumatic stress disorder (PTSD), major depressive disorder, traumatic brain injury (TBI), Alzheimer's disease, and paranoid schizophrenia. Despite these diagnoses, there was no evidence in the resident's medical record of a completed PASRR Level I or Level II screening prior to admission, nor was there a current PASRR assessment available.
