Puree Diet Foods Prepared Without Following Recipes

Summary

The facility failed to ensure puree foods were prepared in the proper consistency and according to facility recipes for residents receiving puree diets, affecting four residents identified as receiving puree textures. During observation of puree hamburger stroganoff, staff made about five to six servings using approximately 34 ounces of ground beef with an unmeasured amount of beef broth and later added unmeasured thickener. The mixture still had visible chunks and small distinct pieces when tasted, and the Regional Manager confirmed it needed to be blended more. The cook stated staff were using only the menu with serving sizes and had not been using a recipe until the Regional Manager printed one and provided it to the cook, who did not review or use it. Additional observations showed similar preparation issues with puree broccoli and puree noodles. For the broccoli, staff placed an unmeasured four to five cups of broccoli in the mixer and added unmeasured broth and thickener, with no recipe or instructions observed during the process. For the noodles, staff placed about 20 ounces of cooked noodles in the mixer and added unmeasured whole milk and thickener, again without using a recipe or instructions. The facility recipe for each item directed staff to blend until smooth and described the expected pudding or soft mashed potato consistency, while the facility policy required food to be provided in the appropriate form and with the appropriate nutritive content as prescribed. The Manager and Regional Dietician acknowledged the puree was not made to the correct consistency and that recipes were not reviewed or used.
