Restorative documentation for March 2026 showed the resident refused services 17 times on day shift and 10 times on evening shift, but the records did not identify reasons for the refusals, resident concerns, or attempts to modify the program to improve participation. Documentation for completed services also did not include the amount of time services were provided, making it impossible to determine whether the program was effective in maintaining, improving, or declining the resident’s functioning.
